25. The reporter says that an investigation needs to show whether the door was dysfunctional or never installed. Sadly there is a third possibility. If the function of the door was overridden by a resident, or if the functioning of the door was disabled, that needs to be investigated as well. I've lived in a building that had these types of doors, and some people who can't stand the way the door automatically closes and slams will actually disable that function, so that they can close the door when they want or leave it open at their own option.
